Key Replacement Form

Important: Missing keys must be reported to the Facilities Department (facilities@fie.org.uk) as soon as possible. Keys will be ordered and cut once the appropriate key donation is made to: Cancer Research UK Please forward your e-mail receipt to Facilities in order to initiate the key cut process.

**Please note, the key replacement process can take up to three working days.

Please ensure you have searched thoroughly for your keys, as reimbursements are not possible once the keys are cut. In the event, you locate your keys after a new cut, please hand them into FIE to be destroyed for security reasons.

All keys are on loan and remain FIE’s property.
